The following forms are available to allow for smooth processing of your membership application. All forms are available in Adobe Acrobat (pdf).

FoRm Description
Affiliate Form Use this form if you wish to join the Institute as an Affiliate member. All other grades of membership are obtained by completing the relevant qualification.
Reinstatement Form Use this form if you were previously a member of the Institute and wish to rejoin.
Fellows Nomination Form Guidelines for Fellowship nomination are included within the nomination form.
Direct Debit Form The annual collection of your subscription can be taken by direct debit (UK based members only). Please download this form to set up a direct debit.
